Founded in 1974, and formerly known as the Joseph M. Smith Community Center, we are committed to our mission to improve the health and well-being of the communities of Allston, Brighton, Waltham, and surrounding areas by providing quality, comprehensive, coordinated care that is patient-centered, family-friendly, and community-focused. As a Patient-Centered Medical Home, we are committed to providing patients with timely access to the right care, at the right place, and at the right time, collaborating with other organizations to connect patients with a comprehensive range of services and provide continuity of care, and creating new community partnerships to meet the changing needs of patients and the community. Smith Building at 495 Western Avenue, offers a full range of clinical services that include primary care, dental, mental health, podiatry, vision, obstetrics, and pharmacy, and many supportive services. Our Waltham Health Center, located at 564 Main St, also offers medical, dental, behavioral health, OBGYN, pharmacy, health benefit enrollment, and community health services. We also operate two school-based sites in Boston Public Schools.
