Hours/Location
As a hybrid opportunity, the position entails onsite coverage 4 days per week (days can be flexible dependent on candidate’s schedule) and remote coverage 1 day per week. Our social workers should be able to work on-site in any of the following locations.
- Beverly
- Danvers
- Lynn
- Marblehead
- Peabody
- Rowley
- Salem
- Saugus
- Swampscott
Due to the hybrid nature of the role, we ensure that our employees receive required technology and training to be proficient and independently productive in all job responsibilities regardless of work location. Employees are responsible for designating a workspace within the remote work location that is private, safe, ergonomic, and free from distractions for all hours worked.
The Opportunity:
General Summary
The Behavioral Health Navigator reports directly to the Social Work Department Team Lead and serves as an integral member of the Social Work Team. This role works closely with clinical social workers, physicians, advanced practice providers, nurses, and other clinical and administrative staff to enhance the delivery of patient care services along the continuum of care. The Behavioral Health Navigator helps support and guide patients through complex social service systems with a goal of promoting social and emotional well-being. With direction from clinical social workers, the Navigator will assist patients in identifying and acquiring quality, culturally competent, behavioral health resources.
Primary responsibilities to include:
- Engages patients in the management of their behavioral health conditions through goal setting, coordination with community services and providers, and referral services.
- Partners closely with clinical social workers and other members of the clinical team to support care plan
- Identifies and removes barriers that interfere with patient access to behavioral health resources.
- Handles follow-up contact with patient/family, care team, and community agencies.
- Promotes an environment that is sensitive to the culturally diverse needs of patients, families and employees.
Research resources and programs available to patients, including but not limited to:
- Individual, group, and family therapists,
- Social opportunities and peer support,
- Behavioral Health Self-Management Tools
As a member of the Care Coordination Team:
- Advocates for the patient to ensure quality outcomes.
- Participates in the planning of care for assigned patient population.
- Identifies and uses appropriate community resources to optimize patient outcomes.
- Finds opportunities for service line process improvement.
- Serves as a resource to the interdisciplinary team for community based social service agencies.
- Maintains effective working relationships to work collaboratively.
- Communicates and collaborates with team to ensure that care coordination needs of assigned patients are met without redundancy of efforts and there is adequate documentation in the patient’s medical records.

- Bachelor’s degree.
- At least 2 years of related work experience required.
- Experience in human services, social services, and/or behavioral health care is preferred.
