GLFHC is seeking a Community Health Worker/SDOH Data Entry Specialist to join our Community Support Services team. Reporting to the CSS Social Services Supervisor, this position is responsible for conducting Social Determinants of Health (SDOH) screenings with patients while ensuring the accurate, timely entry, validation, and maintenance of SDOH data within the Electronic Medical Record (EMR). The Community Health Worker/SDOH Data Entry Specialist supports care coordination by identifying patient social needs, documenting findings, facilitating referrals, and contributing to population health initiatives that improve access to care and community resources.
Responsibilities include:
- Conduct Social Determinants of Health (SDOH) screenings with patients in person, by phone, and through outreach activities.
- Engage patients in culturally responsive conversations to identify social needs and connect them with appropriate resources.
- Accurately enter and maintain SDOH screening results, patient demographic information, and referral documentation within the EMR and other tracking systems.
- Review documentation for completeness and accuracy while identifying and resolving missing or inconsistent information.
- Perform routine data quality audits to ensure compliance with organizational standards and reporting requirements.
- Assist with compiling reports related to SDOH screenings, referrals, program outcomes, and productivity metrics.
- Collaborate with Community Health Workers, care coordinators, and clinical staff to ensure timely documentation and completion of screenings.
- Maintain confidentiality and ensure compliance with HIPAA and organizational policies regarding patient information.
- Participate in Mobile Market registration and other community outreach initiatives by entering patient demographic information and documenting SDOH screening results.
- Support quality improvement initiatives and perform other related duties as assigned.
Qualifications:
- High school diploma or GED required.
- Associate's or Bachelor's degree in Health Administration, Public Health, or a related field preferred.
- Minimum of 1–2 years of data entry experience required.
- Experience in healthcare, public health, or social services strongly preferred.
- Strong attention to detail with a high level of accuracy.
- Proficiency in Microsoft Office (Word, Excel, and Outlook).
- Experience working with EMR/EHR systems or data tracking platforms preferred.
- Excellent organizational, time management, written, and verbal communication skills.
- Ability to efficiently manage high-volume data entry while maintaining accuracy.
